The Health Disability Activity Pathway is designed to empower individuals with disabilities to engage in more physical activities. It has been developed to assist health professionals in directing these individuals toward opportunities for physical activities and sports within their local communities. The pathway is accessible to everyone; you don’t need to be a health professional to use it. Additionally, in-house training is available for your NHS team to deepen their understanding of the pathway and improve their ability to direct individuals to it.
The first step begins with the patient who will contact their local Healthcare Professional. The patient will then be referred to the National Exercise Referral Scheme (NERS) and complete the 4-16 week programme, or they may be deemed that they can advance straight to completing the Health Disability Activity Signposting Form.
The patient’s Local Sports Development Team will then contact them or their parent/guardian to discuss their options. This will likely be signposting to suitable clubs and local sports sessions, or helping the patient to get a leisure card providing access to local health facilities.
Training is accessible to everyone in Wales who plays a role in the care or treatment of individuals with disabilities.
